Goal: The Illinois WISEWOMAN program (IWP) aims to lower heart disease and other chronic disease risk factors through screening and lifestyle classes for women in high-risk populations in service counties throughout Illinois.

Impact: The Illinois WISEWOMAN Program addresses the disproportionate risk of cardiovascular disease among disadvantaged, low-income women. Participation in the program has been shown to improve dietary, physical activity and cardiovascular outcomes.

Goal: The mission of HKES&I is to increase access to needed healthcare for children. This is accomplished through the goal of identifying children who need further follow-up, particularly for hearing and vision problems, and to connect them to those services.

Impact: Healthy Kids Express Screening and Immunization program is successful in providing evidence based care to low-income populations and for connecting those identified with screening needs to needed follow-up care.

Goal: The objectives of the Cardiovascular Disease Risk Screening and Education program are as follows:

-To identify and provide early intervention for those with or at risk for untreated or uncontrolled hypertension
-To screen and identify those at risk for cardiovascular and renal disease
-To educate those found at risk for CVD
-To provide early chronic disease intervention
-To facilitate access for long term chronic disease care with partner sites

Goal: The mission of the Healthy Kids Express Asthma Program is to provide quality asthma care and education to children with asthma and their caretakers in a school environment to work towards goals set by Healthy People 2020 and the National Asthma Education and Prevention Program (NAEPP).

Impact: The Healthy Kids Express Asthma Program serves approximately 600 children a year, improving inhaler technique and asthma knowledge among participants. Children enrolled in the program for two consecutive years have lower school absentee rates and hospitalizations due to asthma.

Goal: The mission of the RPA is to improve the quality of life and the economic competitiveness of the 31-county New York-New Jersey-Connecticut region through research, planning, and advocacy.

Goal: The Coterie exists to provide professional classic and contemporary theater, which challenges the audience and artist alike, and to provide educational, dramatic outreach programs in the community. The Coterie seeks to open lines of communication between races, sexes and generations. The objectives of DHEP are to improve knowledge regarding HIV and STDs and reduce the intentions of teenagers to practice high-risk behavior. DHEP also continues to refine the culturally-specific dramatic workshops and ensure story lines are relevant to teenagers.

Impact: After watching this program, over 75% of students report that they would be less likely to practice high-risk sexual behavior.
